HHCPC's Monthly General Meeting 

Please come join our monthly general meetings on the first Thursday of each month from 6pm-7pm CST!

 

At every meeting, we update all HHCPC general members about the activities we host and opportunities offered from the coalition or other organizations. We will post up the meeting minutes from every meeting below so those who could not attend the meeting can be caught up with the the Coalition has been doing.

COVID-19 Outreach Team (CORT) Updates

  • Ramsey County COVID-19 Racial Equity and Community Engagement Response Team (RECERT) grant ongoing

    • June 2021 – August 2021

  • Survey complete

    • 302 submissions (Currently analyzing data)

  • Public Service Announcements

    • 6 released on the HHCPC Facebook page

    • Will be added to the hmonghealthcare.org website at a later date

  • CORT website will be updated by end of August 2021 (www.hmonghealthcare.org)

June 2021 General Meeting Minutes

Announcements: 

 

  • 10,000 Families Study

    • Hosting a live facebook panel to answer questions about COVID-19 vaccines and discuss what they are doing in the community

    • Starting a pilot project to develop school curriculums on stroke education

    • Selecting about 3-4 middle or high schools with high amount of minority students (Hmong, Somali, Hispanic, African American)

COVID-19 Outreach Team (CORT) Updates

  • Currently working on a grant to provide vaccine education/awareness to impacted communities 

  • Meetings are every Wednesday 6-7pm (except the 1st wednesday of the month) 

 

Flu Clinic Updates

Guest speakers: 

  • Dr. Michael Rosenbloom, MD - A behavioral neurologist who graduated from the University of California, San Francisco fellowship program in 2008. He has been working at HealthPartners in St. Paul, MN for 11 years. He has served as the clinical director of the HealthPartners Center for Memory and Aging, a clinic that focuses on dementia care for 10 years and has been the Department Chair for Neurology since 2015. He exclusively manages persons with dementia and is also performing research in Alzheimer’s disease. He is the director of the Care Ecosystem Program, a grant support effort designed to help provide better support for caregivers of persons with dementia from underserved areas within the Twin Cities as well as throughout the state of MN. Dr. Rosenbloom is here tonight as a result of his interest in learning more about the needs of the Hmong community with respect to dementia care.

May 2021 General Meeting Minutes 

Announcement: 

  • 2021-22 Co-Chair has stepped down their position

  • Position will remain vacant; no future plans to re-elect new board member

  • General meeting minutes will be posted on the coalition website from now on


COVID-19 Outreach Team (CORT) Updates

  • Created during the beginning of the pandemic 

  • Purpose: Provide COVID information and resources to the Hmong community 

  • No meeting yet however email will be sent out to figure out 1st meeting 

  • Grant opportunities opening up to assist with efforts 

 

Flu Clinic Updates

  • Currently in communication with CSE for partnership/collaboration 

  • A flu clinic info page will be created and added on the coalition website 

 

Mentorship Program

  • Application opens next week 05/102/2021 (tentatively) 

  • Healthcares, health students, graduate students may apply to be mentors (eligibility and more details are listed on the application) 

 

Guest speakers: 

  • Linda Vue is an Operations Coordinator at Trellis. She has a background in Gerontology and working with older adults.

  • Mayla Yang is a Community Outreach Specialist at Trellis with a background in human services. She has been working in social services for 4 years. 

  • Xue Yang is a Wellness Engagement Specialist at Trellis. He helps connect older adults to healthy living promotion classes. 

 

Topic: Opportunities and available resources for older adults at Trellis 

  • Trellis is a nonprofit organization that provides innovative services, information and connections that address the changing needs of aging adults in our community. Linda Vue, Operations Coordinator; Mayla Yang, Community Outreach Specialist; and Xue Yang, Wellness Engagement Specialist are all members of Trellis and have collective backgrounds in gerontology, human services, and community health.
