HHCPC's Monthly General Meeting
Please come join our monthly general meetings on the first Thursday of each month from 6pm-7pm CST!
At every meeting, we update all HHCPC general members about the activities we host and opportunities offered from the coalition or other organizations. We will post up the meeting minutes from every meeting below so those who could not attend the meeting can be caught up with the the Coalition has been doing.

Dr. Michael Rosenbloom, MD - A behavioral neurologist who graduated from the University of California, San Francisco fellowship program in 2008. He has been working at HealthPartners in St. Paul, MN for 11 years. He has served as the clinical director of the HealthPartners Center for Memory and Aging, a clinic that focuses on dementia care for 10 years and has been the Department Chair for Neurology since 2015. He exclusively manages persons with dementia and is also performing research in Alzheimer’s disease. He is the director of the Care Ecosystem Program, a grant support effort designed to help provide better support for caregivers of persons with dementia from underserved areas within the Twin Cities as well as throughout the state of MN. Dr. Rosenbloom is here tonight as a result of his interest in learning more about the needs of the Hmong community with respect to dementia care.

Mentorship Program

Application opens next week 05/102/2021 (tentatively)

Healthcares, health students, graduate students may apply to be mentors (eligibility and more details are listed on the application)

Linda Vue is an Operations Coordinator at Trellis. She has a background in Gerontology and working with older adults.

Mayla Yang is a Community Outreach Specialist at Trellis with a background in human services. She has been working in social services for 4 years.

Xue Yang is a Wellness Engagement Specialist at Trellis. He helps connect older adults to healthy living promotion classes.

Trellis is a nonprofit organization that provides innovative services, information and connections that address the changing needs of aging adults in our community. Linda Vue, Operations Coordinator; Mayla Yang, Community Outreach Specialist; and Xue Yang, Wellness Engagement Specialist are all members of Trellis and have collective backgrounds in gerontology, human services, and community health.
